A biofilm is an aggregate of microorganisms in which cells adhere to each other on a surface. These adherent cells are frequently embedded within a self-produced matrix of extracellular polymeric substance (EPS). Biofilm EPS, which is also referred to as slime (although not everything described as slime is a biofilm), is a polymeric conglomeration generally composed of extracellular DNA, proteins, and polysaccharides. Biofilms may form on living or non-living surfaces and can be prevalent in natural, industrial and hospital settings.[1] The microbial cells growing in a biofilm are physiologically distinct from planktonic cells of the same organism, which, by contrast, are single-cells that may float or swim in a liquid medium.

What does this mean to dentistry? Well, it’s time to get real. Biofilm is natural in our daily lives. Every drinking fountain, ice maker, air conditioning system, any where there is moisture and a dark environment (think about your compressor).

Biofilm spores are continuously being emitted from your handpiece spray, air/water syringe & Cavitron. That’s why quality masks are so important (O.S.H.A.).

Most normal healthy individuals have no issue with biofilm; however, people whose immune systems are compromised can be a greater risk for post-op infection. In the public water supply, the CDC has determined that a biofilm level of 200ppm is safe. That’s the water that comes through your kitchen sink. But, in a dental unit, the amount of water that flows through on a daily bases is so small and the water moves very slowly, it allows for biofilm colonies to develop and adhere themselves to the in-side of water lines. This multiplies the biofilm level (at the handpiece) several thousand ppm. Plus, they tend to clog handpiece water lines.

The clean water bottle that is attached to your unit is NOT preventing or controlling biofilm. As a matter of fact, most offices don’t even use them correctly. The purpose of a clean water bottle is to introduce the disinfectant to the inside of the dental unit. The techniques used to clean your lines is at your discretion, however, it is important to do something.

So, if biofilm is so prevalent in nature, how do we prevent it? We can’t, we can just control it. There are several methods commonly used, but remember, you have to use them according to directions for the maximum effect.

  1. High Level disinfectants should be used weekly. This is a maintenance process that does require some minor staff training and the introduction of a new scheduled protocol. Sounds tough, but it’s not and it is the best. At the end of the week, the units are flushed with disinfectant (5 min/op). The residue solution remains in the unit over night (or weekend). Upon returning to the office, the units are flushed with clean water to remove the remaining disinfectant. The disinfectant is usually a notable color (pink/blue) to identify its presence.
