Digital Impressions & Cad Cam, What You Need to Know!

In August of 2012 one of my newsletter was on the differences between the Cerec System and E4D (https://cliffsnotesblog.me/2012/08/19/cerec-vs-e4d-the-real-story-a-product-review-grandioso-flow-by-voco/) . To this day it is by far the most viewed blog posting of the 237 that I have done, with over 3500 views. I guess it is time to up-date my views on “The Real Story”.

We all know that digital impressions are the future of dentistry and I don’t think that there is one person, patient or clinical staff, which will miss the goop & the goo. The investment in a digital impression system will enhance your practice but you are looking at $25k-$30k in technology so due diligence is required to make sure that your investment fits your practice for the maximum ROI.

With more systems entering the dental market it is important that you understand the differences as well as the monthly operating costs along with the costs of maintenance and up-grades. All of the systems on the market today work well and provide the ability for perfect restorations when used correctly and that is the key, when “used correctly”.

The 2 most important aspect of your investment is the “open platform” and the developer’s ability for continued “R&D”. Your restorations are measured in microns and the direct connectivity of the digital data captured by the intraoral camera and system software is necessary or your investment will be either a dinosaur in 3 years or require additional investments for up-grades.

The professional that takes the time to do due their diligence will get the best ROI on their investment. The initial cost of any one system should not be in the due diligence equation. The overall cost of a system on a daily basis and its ability to grow with future technology should be your biggest question. Discus the purchase price after you have decided which system is best for you.

What to look for:

1. Open platform.
2. Learning curve.
3. Cost of digital transmission.
4. Features and benefits.
5. System portability.
6. Cost of up-grades based on past history.
7. Cost of maintenance based on past history.
8. Developer’s commitment and ability for continued R&D.
9. Cost and reliability of service and support based on past history and vendor ability to coordinate.

   Interdisciplinary Diagnoses of Domestic Abuse!

Abstract:

In all 50 states, physicians and dentists are required to report suspected cases of abuse and neglect to social service or law enforcement agencies. The purpose of this report is to review the oral and dental aspects of physical and sexual abuse and dental neglect and the role of physicians and dentists in eva-luating such conditions. This report addresses the evaluation of bite marks as well as perioral and intraoral injuries, infections, and diseases that may be suspicious for child abuse or neglect. Physicians receive minimal training in oral health and dental injury and disease and, thus, may not detect dental aspects of abuse or neglect as readily as they do child abuse and neglect involving other areas of the body. Therefore, physicians and dentists are encouraged to collaborate to increase the preven-tion, detection, and treatment of these conditions.

In one study,12 the lips were the most common site for inflicted oral injuries (54 percent), followed by the oral mu-cosa, teeth, gingivae, and tongue. Discolored teeth, indicating pulpal necrosis, may result from previous trauma.13,14 Gags applied to the mouth may result in bruises, lichenification, or scarring at the corners of the mouth.15

Some serious injuries of the oral cavity, including posterior pharyngeal injuries and retropharyngeal abscesses, may be in-flicted by caregivers with factitious disorder by proxy16 to sim-ulate hemoptysis or other symptoms requiring medical care; regardless of caregiver motive, all inflicted injuries should be reported for investigation. Unintentional or accidental injuries to the mouth are common and must be distinguished from abuse by judging whether the history, including the timing and mechanism of injury, is consistent with the characteristics of the injury and the child’s developmental capabilities. Mul-tiple injuries, injuries in different stages of healing, or a discrep-ant history should arouse a suspicion of abuse. Consultation with or referral to a knowledgeable dentist may be helpful.

Today’s Reality: Everyone is a Leader!

The recent FORUM article, “In the Future, We Will All Have the Chance to be Leaders,” is interesting because of its focus on a predicted future state. I believe that the future is already here, and organizations should proactively consider the impact of their current beliefs and actions on today’s work environment, as well as the future. A few years ago, my wife and I traveled to New York City and stayed at one of the many hotels in Manhattan. As we arrived and every day during our stay, we interacted with several members of the staff, but one individual – a member of the bell services staff – stood out.

This gentleman always seemed to anticipate our needs; he made the extra effort at each encounter to ask us if our stay was still wonderful; and he always seemed to know who on the team to go to for answers or to get things done. On our last day in town, I told him he should work for Disney someday. His response:

“I did work for Disney, on the College Program at Walt Disney World. And, I’ll never forget what I learned there.” Paraphrasing: “Two things: Treat everyone as though they are guests and the work is easier and a lot more fun when we all work together.”

As technology continues to evolve health care professionals are not just accessing health information on desktop computers, the data is mobile.

Here are some tips for protecting health information when using a mobile device, such as a cell phone, tablet or laptop computer. The tips are based on information provided by the federal government, through the Office of the Chief Privacy Officer. While these tips are not exhaustive or definitive, and compliance with them does not necessarily make someone compliant with the Health Insurance Portability and Accountability Act Security Rule, they may help protect patient information on mobile devices.
